Please help us care for your child and notify us of the following:

ANY chronic or acute illnesses.
ANY new medications or changes in medication dosages to be given at school.
Exposure to a communicable disease including chicken pox, conjunctivitis, or strep throat.
A life threatening or new allergy.
A serious injury requiring hospitalization.
Diagnosed with a concussion.
An injury resulting in a splint, cast, mobility assistance (crutches/wheelchair), or stitches.
If your child needs medication at school, this form must be filled out by the parent and the physician.
