You will find information in the links below.

To enroll your child into Preschool you must have the following:

  1. Your child must be 4 years old prior to July 1 to apply.

  2. Provide a record of complete medical immunizations.

  3. Birth Certificate- it can be obtained through the Office of Vital Statistics. Click for the Office of Vital Statistics.

  4. Child well check form

  5. Dental Exam form

  6. Proof of residency- utility bill, etc.

Pre-K Hearing, Vision, and Speech Screenings

Lewis County Schools will be conducting hearing, vision, and speech and language screenings in our schools.
